Use of School Facilities Process

- Complete the two-page Application for Use of School Facilities.
- Submit the completed/signed application to the office of the Director of Athletics, Student Activities, and Facilities thirty (30) days prior to the use of the facility–NO EXCEPTIONS! Please Note: submitting an application does not mean that the request is granted. · IF the application is accepted and the date(s) tentatively reserved, the person or group is put on notice that a Certificate of Liability Insurance* is required before a Contract can be generated and sent along with the application to the Business Office for review and final approval.
- If the required documents meet with final approval of the Business Office, all parties sign the Contract, and the use of school facilities is granted by the Ephrata Area School District to the individual or group. (*) Proof of Liability Insurance due 3 weeks prior to event or preferably with application
