The purpose for this form is for you to refer a student that you are concerned about to a Student Assistance Program team member. This can be about any issues that can pose as a barrier to a students success.  This includes, but is not limited to, substance abuse, attendance issues, mental health, etc.

If you believe the student is in need of immediate help because they pose a danger to himself, herself, or others, call 911.  If this concern is after school hours and is about mental health, please contact Crisis Intervention at 717-394-2631
