The Pennsylvania School Health Law require that children attending school in the Commonwealth receive a medical examination at stated intervals.
A medical examination is required on the first entry into school (Kindergarten or Grade 1), and in grades 6 and 11. These grades were selected because they represent critical periods of growth and development in a child’s life. Transfer students and students with incomplete health records shall also be required to have a medical examination.
You have the choice of having this examination completed by your family physician or the school physician. To clarify any question concerning the cost of this physical examination, the examination given at school will be done at the cost of the school district, while those completed by the family physician will be done at the expense of the parent.
A physical examination completed no earlier than 12 months before the first day of the school year is acceptable. Completed forms must be returned by October 30th. Physical forms may also be obtained from the health room.
If the private physical examination form is not completed and returned to the school, your child will be scheduled for a physical examination by our school physician during the school term. Please contact your school office for a physical form. Complete forms must be returned by October 15th annually.
